Please complete and submit the following form if you have purchased one of our gift cards to make a donation in someone else’s name and would like us to send the card directly to them.

If you haven’t already purchased your gift card please do so first and then return to complete your form. Please note: If you wish to send multiple gift cards to different people, please complete an individual transaction and order form for each recipient.
